These forms are to be used to document the various milestones in your graduate career.
Please DO NOT alter the spacing of the lines because the Graduate School
will not accept these forms if they are altered from the original configuration.
ALL forms are to be submitted to the GRADUATE PROGRAM DIRECTOR who will forward them to the Graduate Office for approval.
Description of Forms:
Use this form to summarize each graduate advisory committee meeting you have.
Use this form when you have selected a laboratory to work in. Together with your Major advisor, you will select an advisory committee composed of your Major Advisor (Chair), two departmental members, and one external member.
Use this form to request an alteration to the members of your PhD advisory committee.
Use this form to request an alteration to the members of your PhD research committee.
These are guidelines for the Mock grant proposal exam given to students at the end of their second year of graduate studies.
These are guidelines for the oral thesis proposal given by students generally by the end of their 3rd year of graduate studies.
Use this form to confirm the completion date of qualifying exam part II: Oral Thesis Proposal. This form is used to report the final decision of the qualifying exam and to inform the Graduate Office regarding the decision of the student's advisory committee regarding advancement to candidacy.
If a student chooses Preparing Future Faculty (PFF) as the teaching experience, the student and mentor will complete the appropriate sections on this form and forward the signed form to the Graduate Director. If a student chooses a formal teaching experience in the classroom, the instructor of the course that the student taught in will confirm participation using this form.
This form is to be completed by the instructor of the course in which a student has chosen a formal teaching experience. This form is for feedback and evaluation of a student's performance in a formal teaching setting.
Use this form when you have passed your advancement to candidacy exam. Together with your Major advisor, you will select a research committee composed of your Major Advisor (Chair), two departmental members, and one external member. This form MUST be submitted together with the Nomination to Candidacy form.
Use this form when you have passed your advancement to candidacy exam. BE SURE to include copies of your mentor’s animal (IACUC), biosafety (IBC), human (IRB), or radiation safety recent APPROVAL together with your research abstract. The approval form must show a current date showing they are approved to conduct the studies you will use in your thesis work.

This form needs to be completed and submitted to the Graduate Program Director at least 30 days in advance of your thesis defense date. This is a public announcement of your defense and cannot be changed. So confirm the date, room, and time before submitting the announcement.
